School Field Trip Reservation Form
Please fill in all fields of this form, so we may know how best to prepare for your student's visit to Rio Grande Valley Maize. The required fields are mandatory. The verify box is a method to limit access by hackers and spammers. The verification code is also mandatory.
A non-refundable deposit will be required upon confirmation of the scheduled date of the field trip. The deposit amount will be determined by the number of students attending. Any changes or cancellations need to be made 2 weeks prior to the field trip, or sooner, so the available date can be used by other groups, or the deposit will be lost. If the field trip is rained out, every effort will be made to reschedule.
